The Eloquent Physician® is an Electronic Charting program which can be utilized to produce physician’s charts with amazing speed in many clinical settings.

An EMR (Electronic Medical Record) program is utilized to produce medical records in a physician's office, clinic or hospital based setting. Some EMR software programs contain advanced features such as electronic data exchange (EDI) between the EMR and a laboratory, radiology facility and/or pharmacy.

The Eloquent Physician does not contain these more advanced features. However, if your main interest is in producing ‘Eloquent’ medical records rapidly, you owe it to yourself to review the features of this program.

General examination of the knee shows {no | mild | moderate | severe} effusion. The patient is able to move the knee within the following range: {- 20 | - 15 | - 10 | - 05 | 0 | 05 | 10 | 15 | 20 | 25}°/{- 20 | - 15 | - 10 | - 05 | 0 | 05 | 10 | 15 | 20 | 25}°/{-20 | -15 | -10 | -05 | 0 | 05 | 10 | 15 | 20 | 25 | 30 | 35}° with the first number indicating hyperextension, the second number active extension, and the third number flexion. There is {no | mild | moderate | severe} tenderness to palpation on the {medial | lateral | medial and lateral} joint line. There is a {negative | positive} deep flexion test. There is a {negative | positive} meniscal click. Instability testing of the knee reveals a {negative | grade one | grade two | grade three} Lachman test, and a {negative | grade one | grade two | grade three} pivot shift test. The posterior drawer test is {negative | positive}. At full extension there is {one | two | three | four | five | six | seven | eight | nine | ten | eleven | twelve | thirteen | fourteen | fifteen} mm of medial joint line opening and {one | two | three | four | five | six | seven | eight | nine | ten | eleven | twelve | thirteen | fourteen | fifteen} mm of lateral joint line opening. At 20 degrees of flexion, there is {one | two | three | four | five | six | seven | eight | nine | ten | eleven | twelve | thirteen | fourteen | fifteen} mm of medial joint line opening and {one | two | three | four | five | six | seven | eight | nine | ten | eleven | twelve | thirteen | fourteen | fifteen} mm of lateral joint line opening. The patellofemoral examination of the knee reveals {no | mild | moderate | severe} crepitation. There is {no | mild | moderate | severe} compression pain of the patellofemoral joint. There is {25% | 50% | 75%} medial translation of the patella and {25% | 50% | 75%} lateral translation of the patella. The Q angle is {5 | 10 | 15 | 20 | 25 | 30 | 35 | 40 | 45 | 50 | 55 | 60 | 65 | 70 | 75 | 80 | 85 | 90 | 95 | 100 | 105 | 110 | 115 | 120 | 125 | 130 | 135 | 140 | 145 | 150 | 155 | 160 | 165 | 170 | 175 | 180} degrees.
