The Eloquent Physician® is an Electronic Charting program which can be utilized to produce physician’s charts with amazing speed in many clinical settings.

An EMR (Electronic Medical Record) program is utilized to produce medical records in a physician's office, clinic or hospital based setting. Some EMR software programs contain advanced features such as electronic data exchange (EDI) between the EMR and a laboratory, radiology facility and/or pharmacy.

The Eloquent Physician does not contain these more advanced features. However, if your main interest is in producing ‘Eloquent’ medical records rapidly, you owe it to yourself to review the features of this program.

(firstName) presents to the office today for a follow-up evaluation of (his_her) lumbar spine. Treatment has consisted of {general therapy and anti-inflammatory medications | chiropractic therapy | discussion regarding possible surgical options | epidural steriod injections | supervised physical therapy | trigger point injections | utilization of an air form lumbar support brace for six hours each day | utilization of anti-inflammatory medications} and (he_she) has responded {minimally | moderately | significantly | well | terribly | desirably | favorably} to the prescribed modalities. (He_She) has no new complaints or issues to discuss at this time. PHYSICAL EXAMINATION: On inspection of the lumbosacral region there is no evidence of deformity. On palpation of the paralumbar musculature, spasm is noted in the lower lumbar region. The patient's range of motion is observed to be diminished in comparison to the normal values in the current AMA Guidelines. The deficiencies in motion are consistent with (his_her) previous examination. Motor strength testing is symmetrical in both lower extremities with no gross weakness detected. Reflexes are equal in the quadriceps and Achilles tendons bilaterally. The patient's gait is observed to be non-antalgic and the patient is able to walk on (his_her) toes and heels without assistance. With the patient in a seated position, a straight leg raise test was performed with no evidence of sciatica. The same maneuver was performed in a supine position, and again no evidence of irritation was noted at the sciatic notch. Sensory examination is determined to be within normal limits in both lower extremities. ASSESSMENT: Lumbosacral Sprain-Strain. Plan: I have recommended that (he_she) continue with {utilization of anti-inflammatory medication as directed | general conditioning exercises at home | supervised therapy as directed | supervised therapy and anti-inflammatory medication as directed | utilization of the air form lumbar support brace as directed | utilization of Natural Relief 1222 | utilization of the home cervical traction unit}. Future treatment considerations will consist of trigger point injections, additional diagnostic studies, and possible changes to the therapeutic modalities. (firstName) has been asked to return for a follow-up evaluation in {1 week | 2 weeks | 3 weeks | 4 weeks | 5 weeks | 6 weeks | 7 weeks | 8 weeks}.
