The Eloquent Physician® is an Electronic Charting program which can be utilized to produce physician’s charts with amazing speed in many clinical settings.

An EMR (Electronic Medical Record) program is utilized to produce medical records in a physician's office, clinic or hospital based setting. Some EMR software programs contain advanced features such as electronic data exchange (EDI) between the EMR and a laboratory, radiology facility and/or pharmacy.

The Eloquent Physician does not contain these more advanced features. However, if your main interest is in producing ‘Eloquent’ medical records rapidly, you owe it to yourself to review the features of this program.

Examination of {the left ankle | the right ankle | both ankles} within normal limits. There is satisfactory plantar and dorsiflexion as well as inversion and eversion. No crepitus is noted with motion. There is no evidence of arthritis or of trauma. The skin is intact with no skin lesions, cellulitis, ecchymosis, swelling or inflammation being noted. No scars are present. There is no tenderness noted over the head of the talus the medial malleolus, lateral malleolus or Achilles tendon. The Achilles tendon is intact and there is adequate pull through of this structure. The patient is not wearing a cast or other type of device on the ankle. There is no evidence of snapping peroneal tendons. (His_Her) ligaments appear to be intact with no evidence of instability present. Specifically the anterior talofibular drawer is negative and the deltoid ligament appears to be intact.
